New Client Forms

​New clients scheduling Shamanic Energy Sessions are required to complete the Client Intake and Liability Waiver forms.

Please take a few minutes to complete these forms prior to your initial appointment. Forms are available via the by clicking the form button. Read below for some additional instructions on the forms.

Client Intake Form: The Client Intake Form is an online form. Complete each section, select the submit button, and your information will be submitted online. Upon submission, you will also receive a completed copy of the form to the email address you included on the form.

Liability Waiver Form: The Liability Waiver form is an online form. Review the form thoroughly then complete the Client Authorization & Signature section, select the submit button, and your information will be submitted online. Upon submission, you will also receive a completed copy of the form to the email address you included on the form. You may download or print a blank version of the form for your records here.
